This unit appears to have been assembled by California Hydronics Corp

Control panel made by Moeller, NEMA type 3R enclosure (see nameplate picture)
Control is 120v, supply is 208v/3ph. Connectors appear to be for 4" pipes
There are 2 additional regulators included.
The unit is mounted on two 4x4's. This unit comes with installation instructions and wiring diagram.
